Outcome measures software for therapists
Send standard measures and your own check-ins to clients, have them scored for you, and see how each client is doing over time.
Built-in outcome measures
These come with Clarity, ready to send:
- PHQ-9, for depression, scored 0 to 27
- GAD-7, for anxiety, scored 0 to 21
- CORE-10, for psychological distress, scored 0 to 40
- PHQ-15, for physical symptoms, scored 0 to 30
Each one is scored as soon as the client sends it, and shown with its severity band.
Send a measure in a few clicks
- Go to Forms, pick a measure and choose Assign clients.
- The client gets an email and a notification, then fills it in from their portal.
- Check-ins can repeat daily, weekly or fortnightly.
- Offline clients can’t fill in forms until they join. Anything you assign is waiting for them when they do.
Risk items flagged for you
If a client scores above zero on PHQ-9 item 9 or CORE-10 item 6, the questions about self-harm and ending their life, you get a notification in Clarity and the result is marked as a flagged risk item.
These alerts appear in the app only, not by email. They support your safeguarding process; they don’t replace it.
Your own check-ins and forms
- Build your own check-ins, outcome measures and feedback forms.
- Questions can be a number scale, free text or multiple choice, with a score for each option.
- Every practice starts with a weekly check-in you can change.
Follow progress over time
- Each client’s page shows their latest score out of the maximum, with its severity band and every answer.
- Clients see their check-in scores as a chart on their own dashboard.
- In Reports, chart scores across your practice, such as the average score for each form or a breakdown by severity, by week or by month.
- Add form results to a client’s PDF export.
