Session notes software for therapists

Write your notes beside each session, with a template if you want one. Every note is encrypted in your browser before it’s saved, so nobody else can read it, including us.

Notes beside every session

Open a session from your calendar or a client’s page and choose Add notes. A session can have as many notes as you need, and each client has one account summary you can keep up to date.

  • Start from a SOAP, DAP or BIRP template, or a blank note.
  • Use simple formatting: bold, italics, headings, lists and quotes. Switch to Preview to see how it reads.
  • Add a short headline so you can find a note again later.
  • Search, sort and pin notes across all of a client’s sessions.
  • Write notes for offline clients too: people who don’t have a Clarity account.

Encrypted before it leaves your device

Before you write your first note, Clarity sets up encryption and gives you a 7-word code. From then on, your browser scrambles each note before it’s saved.

  • Each note and its headline are encrypted with AES-256 in your browser. Our database only ever holds the scrambled text.
  • Your code is separate from your login password, so a stolen password alone can’t open your notes.
  • Clarity doesn’t keep your code. Store it somewhere safe, such as a password manager or a printed copy: if it’s lost, nobody can recover your notes.
  • Once you’ve unlocked your notes, they stay unlocked in that browser tab until you close it or sign out.

Dates, and which session a note belongs to, aren’t encrypted, so Clarity can show each note in the right place.

Who can read your notes

  • You. Notes can only be unlocked with your code.
  • Not your clients. Clients never see session notes.
  • Not us. Clarity only stores the scrambled text.

Every note you add, change or delete is recorded in your audit log, without the note itself. You can also give clients codenames, so their real names don’t appear on screen while you work.

Working notes, not a medical record

Clarity is for your own working notes about each session. It isn’t designed to hold detailed health records from other providers, such as GP letters, psychiatric reports or medication histories. Keep those in a system made for them.

Take your notes with you

  • Export your whole practice from Settings as a zip file, with a spreadsheet and PDFs. Unlock your notes first to include their text.
  • Export one client’s record as a PDF, and choose what goes in it, including their session notes.

If you ever leave Clarity, your notes come with you.
